Instruction for Filling-up the Form

1. Application can be done only through Madrasah Website: www.nmshm.org

2. Hard copy of the application form has to be submitted at the time of admission.

3. Name, Date of birth, Father's name of the student must be similar in Last attending school record, Birth Certificate and AADHAAR.

4. If any of the above three documents is/are unavailable then application may be done on the basis of available document(s).

5. Date of birth must be between below
VII - 01.01.2013 to 31.12.2013
VIII - 01.01.2012 to 31.12.2012
IX - 01.01.2011 to 31.12.2011

6. Admission Procedure:
a) Number of seats: As available
b) List of eligible candidates and date of admission will be displayed on the Notice Board of Madrasah.

7. Documents to be submitted at the time of admission:
a) Banglar Shiksha Transfer Certificate from Last attending School mentioning admission number & date.
b) Xerox copy of Birth Certificate, AADHAR, Guardian's EPIC, Cast Certificate, Handicapped Certificate.
c) Copy of Blood group Certificate if available.
** Hard copy (Print Out) of Application Form should be subbmitted at the time of admission.

9. Girls students belonging feeder area of NAIMOUZA SUBHANIA JR. GIRLS HIGH MADRASAH will be admitted in the said Jr. Girls High Madrasah on priority basis.
