Instructions for Filling-up the Form

1. Application can be done only through Madrasah Website: www.nmshm.org

2. Hard copy of the application form has to be submitted at the time of admission.

3. Online application: 15/05/2026 to 28/05/2026.

4. Students may fill up more than one form, so any eligible candidate may apply in different form for different combination (for Science Stream only), but after selection no one can change his Subject combination as specified in the selected form. 3. List of Selected Candidates (stream, subject and reservation wise) will be published on in website (www.nmshm.org) and also in the Notice board of the institution. Admission date will be published in Notice Board.

5. During Admission, students have to carry the Self attested Xerox copy of all testimonials and birth certificate along with original copy of the same, one passport size colour photograph and admission fees for different disciplines will be published in Notice Board.

Xerox copy of OBC/BPL (optional).

Xerox copy of Bank pass book (first page).

Xerox copy of Admit card, Marksheet, Aadhaar Card and Guardian Voter ID Card.

N.B.:- During admission have to carry Original Admit Card and Marksheet.
